In order to determine eligibility and complete certification for “serious mental health condition,” as required by federal rules, it is essential for individuals to schedule a Clinical Diagnostic Assessment (CDA) to identify mental health needs
The (CDA) is necessary for FMLA support as it ensures thorough evaluation of the your mental health condition, supporting whether or not you meet FMLA criteria. This enables accurate documentation, informs you of important mental health information, and aligns with professional standards for mental health practitioners. During the CDA, we will discuss your specific circumstances and evaluate your mental health and aspects of wellness
The Family and Medical Leave Act (FMLA) defines a "serious health condition" as an illness, injury, impairment, or physical or mental condition that involves either inpatient care or continuing treatment by a health care provider. This typically includes conditions that incapacitate an individual, rendering them unable to work, attend school, or perform other regular daily activities
If you are eligible for leave as determined by the CDA, I provide a written assessment to you, and complete the FMLA certification documents from your employer

Payment for an FMLA or CDA assessment does not guarantee that I will recommend FMLA leave or complete documentation in support of it. The $350 fee covers only the assessment process, during which I evaluate whether your symptoms meet the criteria for FMLA or other accommodations. My determination is based on clinical judgment and ethical guidelines. If your condition does not meet the necessary criteria, I will not be able to recommend FMLA leave. This ensures that all assessments are conducted fairly, professionally, and in accordance with best practices in mental health care.
